## Membership

- **Price:** $129.00/month
- **Billing:** Monthly, cancel anytime, no long-term contract
- **Who:** Every patient. Membership is required to be seen.
- **Includes:**
  - Primary care video visits, scheduled as clinically needed
  - Behavioral health medication-management follow-up visits
  - Direct access to Ann Bacher, the same clinician every visit
  - No copays, no deductibles, no surprise bills
- **Excludes:** Prescriptions, labs, and imaging (billed by the pharmacy or facility providing them; these may be billed to your insurance)

## Comprehensive Behavioral Health Evaluation

- **Price:** $300.00 one-time
- **Duration:** 60 minutes
- **Eligibility:** Enrolled members only
- **Includes:** Full evaluation, diagnosis, and treatment plan
- **Note:** Ongoing medication management after this evaluation is included in membership at no additional charge

## Wellness Packages (member-only add-ons)

Flat-rate monthly pricing, quoted at the first visit. Available only to enrolled primary care members.

- **Hormone optimization** — men's and women's programs with clinical monitoring
- **Medical weight management** — GLP-1 programs with clinical oversight
- **Peptide protocols** — clinically supervised

## Assisted Living Facility Rounding Service

- **Status:** Launching 2027, not yet available
- **Pricing:** Negotiated per facility
- **Current offer:** No-cost, no-commitment design conversations with facility Executive Directors and Directors of Nursing; free blood pressure screening events for residents of partner facilities

## Payment & Reimbursement

- **Accepted:** Cash, credit card, HSA, FSA
- **Superbills:** Provided monthly on request for possible out-of-network insurance reimbursement
- **HSA/FSA:** Membership fees and visit charges are generally eligible expenses
- **Insurance:** Not accepted. Medicare and additional service lines are planned as credentialing completes.

## Policies

- **Cancellation of an appointment:** Less than 24 hours' notice may be billed at 50% of the visit rate
- **Cancellation of membership:** Anytime, with notice before the next billing date
- **New patients:** Currently accepting adults and older adults. Not accepting pediatric patients.

## Scope notes

- All care is delivered by face-to-face video telehealth. No in-person visits.
- Schedule II stimulant medications for ADHD are not prescribed.
- Marijuana certifications are not provided.
- Patients must be physically located in Florida at the time of the visit.
